目前老年服務科學使用穿戴式的干擾性器具進行「監視性」的服務,因此造成老年人有不受尊重之感,畢竟老年人不等於病人,不是老人服務就非使用身體機能監測機具儀器不可,這是一個需要被關注的議題。

使用一個不令人感到不舒服的服務,運用各項數碼資訊的收集後再進行各項大數據分析,由數字分析得知老年人所需的即時性與非即時性的服務資訊,判斷是否立即提供適切的服務介入或是保有持續服務關注。

這些數據服務如同是守護老年,生命,生活,生存的三生態,恰巧適宜應用具有歷史悠久的古老醫學,這是一個以中醫藥理論與實踐經驗為主體,研究人類生命活動中健康與疾病轉化規律及其預防、診斷、治療、康復和保健的綜合性科學。

中醫四診,古代有「望而知之謂之神,聞而知之謂之聖,問而知之謂之工,切而知之謂之巧。」這四種診斷疾病的方法各有層次的不同。所謂神是「神醫」的意思。

  • 「望」,觀望其臉面部五色及肢體形態,診斷身體各個部位的異常變化來知道他的病情。
  • 「聞」,是聽其聲音及聞其氣味,古代醫家以五音的診法視為聞診的基礎。如在古代的音樂中,宮、商、角、徵、羽代表了聲音的五個音階。將這五個音階應用到醫學並與五行及五臟相對應,以耳朵辨別其變化以辨別他的疾病;聞氣味,病體的氣味與病室的氣味兩種,都是指與疾病有關的氣味而言,以病體的氣味來說,可以透過聞患者的口氣以知道其疾病的狀況。
  • 「問」,是經由口問患者的飲食、起居及疾病的起始、症狀等以知道疾病所起所在之處。
  • 「切」,是經由脈診,腹診,經絡穴位診及按診以知道疾病的寒熱、虛實以及病在那一個臟腑。

上述四種對疾病的綜合診斷方法完全不需要複雜儀器的繁雜檢查方式,卻能直接迅速而全面地對疾病做一個正確地診斷,這就是大數據應用分析的原理,不禁讓我們佩服古人的智慧。

中醫學以陰陽五行作為理論基礎,將人體看成是氣、形、神的統一體,通過望、聞、問、切,四診合參的方法,探求病因、病性、病位、分析病機及人體內五臟六腑、經絡關節、氣血津液的變化、判斷邪正消長,進而得出病名,歸納出證型等,這恰巧的與以大數據分析為老年數碼服務平台理念相互結合,因此筆者以中醫四診為主軸,進行專題探討研究。

「應用中醫四診理論導入居家老年服務平台」,這是一個運用物聯網概念為基礎的核心技術,透過各個精准的數碼集結裝置進行多層次數據資訊收集,再經由具有實質經驗參數來判斷分析,最後獲得服務指令,進而執行指令作業。這個系統可以發展成為一個綜合性服務平台,提供給其他更多的服務系統來使用,可經由本系統平台取得各項數碼資訊,來開發許多老年服務應用產品,如:食物使用服務,能源使用服務,日常用品服務等等,屬於一個延伸性的應用設備產品。

「望」此為觀與視原理,老年需要的是幫助,而不是限制與約束。本系統連結社區攝影機或居家觀護設備裝置,瞭解到老年生活移動情況(當然不涉及隱私部分為原則)。「聞」及「問」合而為之,此為主動關懷為之,由服務中心人員主動以遠端視訊或語音裝置進行互動關懷,用來建立收集參數,以便個案資訊修正,達到準確與正確性。「切」此為動態服務,如切脈般的循動而為之,由編列的服務團隊小組進行走動式到老年居家直接進行有溫度的關懷服務。

整體服務平台使用各項終端資訊裝置分別有:「數碼流量裝置」,「電能數碼傳輸模組」,「用水數碼傳輸模組」,「環境數碼訊息傳輸模組」,「燃料數碼傳輸模組」,「智能馬桶裝置」,「智慧臥墊資訊傳輸裝置」,「智能照明傳送模組」等,及透過區域服務中心、遠端關懷服務中心、數據分析決策中心等建構以安全,安心,貼心的出發點建置設備,24小時全日的資訊集結運行,不斷的進行老年生活的正常維護與異常介入服務。

數碼服務平台已持續長達七年時間運行,針對1000個老年戶,由114,670,871筆數據中,交叉比對出12,586筆數碼資訊,建構溫度與用電及空間行為鏈結分析、時間與用水用電及空間行為鏈結分析等等各種老年行為模型分析總結,得知其每個老年人的生活模式與習慣及異常訊息,透過區域服務中心、遠端關懷服務中心、數據分析決策中心等多層次資訊告警、服務、回報、分析、管理等作為達到預防、多重診斷訊息、遠端基礎緊急治療訊息、康復服務建議和保健訊息提供的綜合性服務。 數碼老人服務是否值得期待?隨著國家進一步放開養老市場,對智能養老的重視程度越來越大,相關信息技術的不斷提升,智能養老的前景愈發光明。當然,想要做到「老有所養、老有所為、老有所樂」,打造數碼養老服務產業的特色養老服務平台,還需要將「寬帶網路+」引入養老服務行業的各個領域,放開性的寬帶網路數碼養老服務模式無疑給養老服務業注入了新的活力。

Four Diagnostic Theory of Traditional Chinese Medicine into Elderly Home Health Digital Service Platform

「Summary」

Currently, the elderly healthcare services use the wearable device to carry out their "surveillance" service, because of its disturbing type of design, the elderly have no sense of respect. After all, the elderly cannot be treated as the patient. Elderly healthcare does not necessarily need to use the physical function monitoring equipment, this issue is indeed need to be concerned more further. The use of a comfortable service, collecting and combining multiple digital information and then analyzing the big data, the final digital analysis will provide the information about the elderly need immediate or non-immediate healthcare service, then determine the need of providing the immediate intervention or keep ongoing service. Those data services, acting as the guard of the old age, its life, live and survival – the three essential elements of ecology, happen to be suitable for the application of a long history of ancient medicine. This comprehensive science employs the theory and the practical experience of Traditional Chinese medicine as the main subject, and studies human life activities in health and disease transformation and its prevention, diagnosis, treatment, rehabilitation and health care.

The four diagnostic methods of the Traditional Chinese medicine, there is an old saying "To know it by observation, it is god; To know it by auscultation and olfaction, it is holy; To know it by interrogation, it is an artisan; To know it by pulse feeling and palpation, it is skillful." These four methods of diagnosis represent different levels. “Observation" is observing the five-color of the face and the shape of the limbs, the diagnosis of various abnormal changes of the body parts to know his condition. “Auscultation and olfaction” is to listen to its voice and smell the smell, the ancient doctors consider the fine notes of Traditional Chinese music are the foundation of the auscultation and olfaction. Such as in the ancient music, the “gong”, “shang”,“jiao”, “zheng”, “yu” represent the sound of the five scales. Apply these five scales to the medicine and correspond to the five elements and the five internal organs, with the ears to identify his changes to determine his disease; smell the smell, the smell of the sick and the smell of the ward, both refers to the disease-related odor, to the smell of the disease, you can smell the patient's breath to know the status of their disease. " Interrogation ", is interrogating the patient's diet, living style and the beginning of the sickness and the symptoms, etc. to know where the disease originated. “Pulse feeling and palpation ", is through the pulse diagnosis, abdominal diagnosis, meridian acupuncture points and palpation to know the cold, heat, deficiency and excess of the disease, and the originated organ of the disease. Not only the above four kinds of disease comprehensive diagnostic methods do not require the complex diagnostic procedure of the complex instruments, but can also directly and quickly and comprehensively do an accurate diagnosis of the disease, which is the principle of analyzing the big data application, we cannot help but admire the wisdom of our ancients.

Traditional Chinese medicine uses yin-yang five elements as its theoretical foundation, regards the human body as the unity of qi, body, spirit. Through the “observation”, “auscultation and olfaction”, “interrogation”, “pulse feeling and palpation”, four diagnostic methods, to explore the cause of the disease, the character of the disease, the location of the disease, and analyze the change of the pathogenesis, internal organs, the meridian and the joints, qi, blood and body fluids, to determine the exuberance of healthy and pathogenic qi, thus identify the diagnosis of the disease and conclude the symptoms of the disease, etc. This concept can combine with the idea of using big data analysis for the elderly digital healthcare service platform, thus the author conducts this designated study by using four diagnostic methods of Traditional Chinese medicine as the main axis of this article

「Introduction」

In recent years, the development of digital services for the elderly is still in the early stage, the application scale has been limited, why? What kinds of development bottlenecks need to be solved?

Digital Service is a big platform setting by the cooperation of the multiple functional systems. However, in the current market, some of the technology and products can only provide single task, the different products between each brand also make it difficult to effectively work with each other, thus resulting in fighting among those manufacturers and increasing market chaos. More importantly, at the design level, due to the lack of top-level design, not having the same standard and enough technology, a set of effective management service platform has not been established, this leads a serious limitation to the development of digital healthcare services for the elderly.

Therefore, many companies pay more attention to technology research and development, spend most of the effort on the product itself, but inadvertently ignore the actual demand for the elderly. Resulting in less real practical products in the current market, even though there are many digital elderly heath care services products.

What kind of digital services do the elderly need? First of all, in order to achieve the convergence between different products, the standards of a unified device interface, data format and transmission protocol need to be established, digital terminal equipment need to be provided. Second, to utilize the platform system and the big data to improve the efficiency of elderly care institutions and maximize the reduction of labor costs, a digital information system platform with efficient management services need to be established and a variety of intelligent devices need to be integrated into the platform.For the elderly home healthcare, we should pay attention to enhance the technological element of the elderly digital service products. In Japan, for example, the government encourages the development of the family life support robots, the elderly daily activities and movement, walking assistance, excretion, loss prevention and warning and some other problems found in the elderly home care can be resolved through the robots.

In summary, this author believes that everyone should be aware of any elderly digital service products need a "experimental field". To achieve the needs of the elderly in line with the needs of the elderly services, the developer should personally get involve with the elderly's daily life, and find out one of the "pain point" problem, understand the actual needs of the elderly.

「Improving the utilization of elderly service resources 」

Elderly care service is a sensor network system and information platform for elderly home healthcare, community and elderly care institutions, and is on this basis to provide real-time, fast, efficient, low-cost, physical, interconnected, intelligent digital services for the elderly. Through the Internet of Things (IoT), Big data, digital terminal equipment and other advanced technology and products instead of traditional manpower, Elderly digital care service not only greatly reduces on the need of the number of the community, institutions service staff to further reduce labor costs, but also effectively meet the need of the majority of the elderly care, to maximize the use of intelligent resources. In fact, some smart technologies and products have been applied to the daily life of the elderly. Such as wearable smart bracelet, to achieve real-time positioning, emergency calls, dangerous warning and other functions; intelligent mattress can be real-time monitoring of the elderly's sleep status and physiological indicators and other data. Children of the elderly or the elderly institutions can utilize these intelligent equipment, keep track of the dynamics of the elderly at any time. But no interfere with their daily life and no monitoring their behavior, to show the respect to the elderly is a must.

「The development of service system platform」

"Apply four diagnostic methods of Traditional Chinese medicine into the elderly home healthcare service platform ", which is a concept based on the use of IoT. It bases on the core technology of IoT, through a variety of precision digital assembly device for multi-level data collection, and then by the actual experience parameters to analyze the data, final service instructions will be instructed and executed. This system can be developed into a comprehensive service platform, available to more other service systems. Through the system platform to obtain the digital information, many elderly service applications can be developed, such as: food use services, energy use services, daily necessities services, etc., these will be an extension application of the products

Through analysis and induction of the computer system, the Four diagnostic methods create a multi-level digital database, as an effective implementation of the instructions and immediate provision of services. This service is divided into "direct intervention services" and "indirect information services". The former is directly providing services for the elderly, the later provides information services for the equipment manufacturers. " Observation " is the concept of observation and inspection, and the principle of the need for the elderly is to help, rather than restrictions and constraints. The system connects with community cameras or home care equipment to understand the movement of the elderly (the goal is to not to invade their privacy). " Auscultation and olfaction " and “interrogation” combine together, mean the active care, the staff from the service center will take the initiative and interactive care by using remote video or voice device, it can also be used to collect the data to establish parameters, so that case information can be monitored and corrected if necessary, to ensure its accuracy. “Pulse feeling and palpation” is a dynamic service, provides the moving, warm and caring health care services as the pulse-like movement by the provision of the service team directly visiting the elderly home.

The overall service platform uses the terminal information devices, namely, "Digital Traffic Unit", "Power Digital Transmission Module", "Water Digital Transmission Module", "Environmental Digital Message Transmission Module", "Fuel Digital Transmission Module" , "Smart Toilet", "Smart Pad Information Transmission Device", "Intelligent Lighting Transmission Module" and so on, and through the regional service center, remote care service center, data analysis and decision center to build a safe, peaceful and caring equipment, 24 hours a day of information collection, and constantly carry out the regular daily life maintenance services and abnormal intervention services for elderly.

The digital service platform has been running for up to seven years, of 1000 elderly households, crosse- comprising over 12,586 digital information, built and analyzed the relationship among temperature, electricity and behavior space, and the link among time, water usage and behavior space, and so on. Finally, the collected information was analyzed and summarized to establish a variety of elderly behavior models, each elderly’s normal and abnormal lifestyle, habits information are saved in the system, via the regional service center, remote care service center, data analysis and decision center, etc. to provide information of alarm, service, report, analysis, and management, etc. The goal of this comprehensive service is to achieve disease prevention, provide multiple diagnostic information and remote base emergency treatment information, rehabilitation services advice and health care information.

「Conclusion」

Is the elderly digital health care service worth looking forward? With the country further liberalizing the elderly market, the importance of the intelligent elderly home health care system is growing, the relevant information and technology continues to improve, the prospect of the intelligent elderly home health care system is getting brighter. If one wants to accomplish “supporting the elderly, making the elderly feel the sense of belonging and happy”, certainly, that we need not only to create a characteristics platform of the elderly digital health care service industry, but also need to introduce "broadband network " into the elderly network industry. An open broadband network for the elderly digital healthcare service is undoubtedly has introduced new vitality into the elderly service industry.

In order to facing the current harsh situation of the elderly healthcare industry, the digital system platform of the Golden Apple Intelligent Technology Co., Ltd., has provided the integrated information aggregator, it will be from the "non-wearable device equipment" and then lately develop into the "wearing equipment", while in the meantime, each district will establish a small "Life science detection device" to improve the platform services.
